Critical incident responders come from a variety of fields. Rescue workers (e.g., police, firefighters, emergency medical professionals) are often the first-line responders. However, the helping professionals and volunteers from emergency response organizations are often just behind the first-line responders, allocating resources, assessing situations, and addressing mental health needs of survivors.

Practitioners respond to a variety of critical incidents, such as natural disasters (e.g., hurricanes, earthquakes, and tornadoes); human-made disasters (e.g., oil spills and chemical accidents); acts of terrorism (e.g., the Oklahoma City bombing and 9/11); crises (e.g., job loss or surviving an airplane crash); as well as other trauma-causing events (e.g., childhood abuse, war, and hostage situations). The roles and responsibilities of responders vary, depending on the specifics of each situation. A helping professional responds differently when working with a natural disaster survivor than he or she would when working with a sexual abuse survivor. In this Discussion, you compare the different roles and responsibilities of responders when responding to various critical incidents.

When a critical incident occurs, it can feel like an emotional earthquake to the survivor, disrupting his or her overall functioning. In cases of prolonged trauma (e.g., childhood sexual abuse or war), there is no immediate “clean-up” effort, resulting in prolonged exposure to trauma. In other cases, such as natural and human-made disasters and acts of terrorism, there are often large-scale responses to intervene immediately following the event. The impact of crisis, trauma, and/or disaster on the mental health of the survivor is dependent upon the type of critical incident, the available resources, the actions of the responders, the physical and emotional state of the survivor, and the survivor’s support systems.

For this Assignment, read the case study Sam located in this week’s resources. Think about the impact of crisis, trauma, and/or disaster on the individual in the case study. Think about a crisis, trauma, or disaster that you have experienced. Reflect on the impact it had/has on your mental health and the mental health of those around you.

Crisis and crisis intervention theory

Understanding crisis and crisis intervention theory is vital to skill acquisition for responders and increases their ability to provide effective responses to survivors. There are three levels of crisis intervention theories. The first and most basic level focuses on correcting temporary affective, behavioral, and cognitive distortions caused by critical incidents. The second, or expanded level, addresses the same components as basic crisis theory and also includes the social, environmental, and situational factors that foster a critical incident. The third, or applied, level encompasses four domains: developmental, situational, existential, and ecosystemic (James & Gilliland, 2013). An understanding of these theories allows you as a practitioner to apply the most appropriate theory when working with a survivor of a critical incident. When choosing a theory, it is important to understand that you do not have to be a purist (i.e., adopt only one theory). In fact, many professionals believe it is important to have a working knowledge of many theories so as to adapt to the needs of the client by pulling from several theories when appropriate.

To understand effective service delivery in response to critical incidents, helping professionals must first understand theories of how individuals process crisis and models for responding to survivors. For example, being aware of the Childhood Sexual Abuse Accommodation Syndrome can help sexual trauma practitioners conceptualize the cognitive processing of sexual abuse from the perspective of the child or adult survivor (Summit, 1992). Other theories include the eco-systemic theory (the effects of crisis affect more than just the survivor), the constructivist self-development theory (each individual constructs the meaning of his or her own traumatic event), and the resilience theory (the ability to recover from crisis events). Models provide techniques for working with crisis survivors, including but not limited to Psychological First Aid and suicide prevention models. Familiarity with theories and models related to crisis and crisis intervention provides important foundational knowledge that informs effective response to survivors.
