Option 3: John Doe, Patient One, is in late stage of kidney disease. If he does not receive a new kidney, then he is predicted to die within a week. Doe is 45, single, and has no children. Doctors theorize that Doe damaged his kidney by not following a low-salt diet. Doe inherited one million dollars and is known for giving money to charity. Without a transplant, he will probably be forced to spend all his money searching for a kidney outside of the usual legal channels. Patient Two is Jane Doe (no relation to John). Patient Two is a mother of two children (ages 21 and 24). She is divorced and 55 years old. She developed kidney problems due to eating a high-fat and high-sugar diet. If she does not receive a kidney within one month, doctors believe she will die. Patient Three is an orphan. This orphan lives in a state facility. She was born with a genetic condition that constantly damages her kidney. The only known approach to her condition is to provide her with a kidney transplant every so often. She is 11 and has already undergone two kidney transplants. She will perish in two months if she does not receive another transplant.

All three patients are at the same hospital. The hospital only has one kidney to give out. The orphan’s birth parents were known to be of a religion that is opposed to organ donation. The other patients come from religions that do not oppose organ donation. Who should get the kidney? Why should that candidate receive it over the others? Devise a course of social action and a solution for this case by using the ethics of egoism and then utilitarianism to a key moral conflict involving health care in this case. Appraise the interests of diverse populations (in terms of ethnicity, race, religion, sexual orientation, etc.) and how they relate to the case. Unpleasant symptoms theory

The role of the nurse provider is to analyze the symptoms of the patients. The theory of unpleasant symptoms posits that symptoms include four dimensions: time, intensity, quality, and distress. Each of these dimensions can be measured. Influencing factors, including physiological, psychological, and situational, can affect the experience of symptoms. Furthermore, the symptoms themselves can affect performance outcomes, such as attention-seeking behaviors or quality of life. (Lenz, E. R., & Pugh, L. C., 2018)

The theory of unpleasant symptoms divides the patient’s symptoms into those that are unpleasant and those that are not. It proposes that the emotional reaction of individuals to a physical injury or illness is influenced by the way in which they interpret the environment (Blakeman, 2019). Therefore, the unpleasant symptoms model should be used during patient assessment because it provides the patient’s symptoms in a holistic way. Therefore, the condition of the patient can be easily identified.

Unpleasant symptoms mainly include mental anguish or physical discomfort. Symptoms always range from mild to severe and should always be documented in detail. The theory focuses primarily on the patient’s symptoms and defines them based on how the patient reports. The symptoms experienced by the patient are always perceived as indicators of change in her state of health, and may differ according to intensity, quality of time and suffering (Gomes et al., 2019).

It is also necessary to evaluate and document the various contributing factors, such as pre-existing medical conditions or lifestyle choices, as well as other elements that could affect symptoms, including; culture, age and gender. The consequences of the symptoms must also be identified.

Other symptoms that may not be uncomfortable should also be addressed. Some examples include; changes in sleep patterns, appetite, and energy levels. It is also vital to take note of any other abnormal behavior that may indicate lifestyle habits and other underlying issues contributing to the patient’s condition (Blakeman, 2019). The symptom can cause changes in functional and cognitive responses.

There is a need to identify any signs of improvement over time. It can play a role in determining the effectiveness of the treatment plan. Through this, healthcare providers can understand the need to change the treatment plan if it is not effective.

“Generation” And “Alphabetical Africa”

Some potential questions to explore in this week’s discussion board:

The “generation” in question in Kwock-Kim’s poem doesn’t seem to be a matter of a historical or biological time-frame–what might the author be referring to here? Why might this piece be broken into numbered sections? What kind of symbolism might numbers like “0” and “1” represent? What other metaphors of birthing does the poet use here?

Walter Abish’s “Alphabetical Africa” gives a brief and obscure account of some of that continent’s colonial history in an odd prose style emphasizing the first letters of its words—the list of questions that accompanies this reading asks us to consider such letters in a more in-depth fashion than we are generally accustomed.

A 39-year-old homeless man presents to the emergency department

A 39-year-old homeless man presents to the emergency department for cough and fever. He says that his illness has been worsening over the past 2 weeks. He originally had dyspnea on exertion and now is short of breath at rest. On questioning, he tells you that he lives in a homeless shelter when he can, but he frequently sleeps on the streets. He has used IV drugs (primarily heroin) “on and off” for many years. He denies medical history but the only time he gets medical attention is when he comes to the emergency department for an illness or injury. On review of systems, he complains of fatigue, weight loss, and diarrhea. On examination, he is a thin, disheveled man appearing much older than his stated age. His temperature is 100.5°F (38.0°C), his blood pressure is 100/50 mm Hg, his pulse is 105 beats/min, and his respiratory rate is 24 breaths/min. His initial oxygen saturation is 89% on room air, which comes up to 94% on 4 L of oxygen by nasal cannula. Significant findings on examination include dry mucous membranes, a tachycardic but regular cardiac rhythm, a benign abdomen, and generally wastedappearing extremities. His pulmonary examination is significant for tachypnea and fine crackles bilaterally, but no visible signs of cyanosis. His chest x-ray is read by the radiologist as having diffuse, bilateral,

interstitial infiltrates that look like “ground glass.”
